Policies and Forms
Here, you will find copies of our policies, procedures, and practice-related documents. Digital versions of these materials, along with the required consent forms, will be available for completion through our patient portal once your appointment is scheduled.
Cancellation Policies
If you need to cancel or reschedule your appointment, please provide at least 24 hours notice. Cancellations or rescheduled sessions made with less than 24 hours notice will be subject to a cancellation fee.

This policy is in place because your appointment time is reserved specifically for you. If you arrive late to a session, the remaining time will be used as scheduled, and the full fee will still apply.

If you do not confirm your appointment in a timely manner, your scheduled time may be forfeited and require rescheduling. However, appointments forfeited due to failure to confirm will not be subject to a cancellation fee. It is the responsibility of the client or responsible party to ensure that our practice has the most up-to-date contact information.
